Integrated Care Management (ICM)

Integrated Care Management, (ICM) is a telephonic clinical program with a collaborative team of both nurses and behavioral health clinicians who support commercial members with a holistic care approach. ICM partners with members to:
1. Further holistic care?assessing the interaction of both physical and behavioral health needs
2. Collaborate with the member's providers and the member's support network
3. Address Gaps in Care
4. Find resources and providers for all of their health needs
5. Assist in transitions and planning from different levels of care
6. Educate on symptoms and coping skills
Integrated Care Management employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues. Identifies and resolves barriers that hinder effective care. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations. May create member care plans. Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas. Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed. Follows established guidelines/procedures.
Required Qualifications
Registered Nurse
Ability to be licensed as a registered nurse in multiple states without restrictions.
Bachelor's degree in Nursing (B.S.N.) and/or Master's degree
4 years or more in an acute care setting of clinical nursing experience which includes at least 2 years in an integrated care setting managing members with both Medical and Behavioral Health needs
Licensed Masters level trained Behavioral Health clinician
3+ years of care management experience which includes at least 2 years' experience in an integrated care setting managing members with both Medical and Behavioral Health needs
Minimum of three years of experience in behavioral health direct patient care
Licensure in field of study within the state individual will be required to practice
Ability to work a minimum of 40 hours per week between the hours of 830am- 9pm EST (at least 2 days a week until 9 PM EST)
Preferred Qualifications
CCM certification required or must obtain within 1 year
Knowledge of community health and social service agencies and additional community resources

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
